No show policy**: $25 for Initial Intake no show or late cancel within 24 hours.
For established patients, a $50 fee will be charged for late cancellations for any reason within 48 hours and the full price of the appointment will be charged for a no show.
For paperwork done on behalf of patients at their request, $25/15 minutes will be charged.
Payment can be made by mailing in a check at this time to Holistic Mental Health, PLLC, 800 West Williams Street, Suite 231A Apex, N.C. 27502.
Payment will be due at the time of the appointment via check or cash. Checks should be made out to Holistic Mental Health, PLLC. A returned or cancelled check will incur an additional $25.00 service charge. I am not accepting credit cards or debit cards at this time. Should payment not be made after two consecutive appointments, payment prior to the third appointment must be made (via mailed or hand delivered check) in order to be scheduled to be seen for the third appointment. A fee service agreement agreeing to these provisions as listed on this page will be signed by new patients prior to the initiation of services and periodically by established patients when changed are implemented which can be done by the patient in the waiting room just prior to an initial intake or before a follow up appointment.
*After-hours phone calls occur on weekends, holidays, and during the week between 6:00 P.M. – 8:00 A.M. The first minutes are free, but for calls running longer than fifteen minutes a $25 charge is incurred. However, if a patient consistently calls the emergency cell line after hours over the course of at least several weeks the initial phone period will be waived and a $25 fee will be charged from the start of the phone call at the discretion of the provider with subsequent minutes also being charged at the rate as listed above.
**There are exceptions to charging for a late cancellation or no show in the case of an emergency. The nature of the emergency as a reason for a missed appointment will be evaluated on a case by case basis and the provider is the final arbitrator of whether a patient will be charged for the missed appointment time.
