Office Visits

Currently I am seeing patients either in person or by telemedicine. For those individuals wanting to do a telemedicine appointment, I am using Zoom, which is a HIPAA compliant telemeeting software for being able to do remote sessions. You may download the Zoom app for free on your phone by going to Google Play or the iOS app store and downloading the "Zoom cloud meeting app" which will allow you to access the telepsychiatry meeting from your phone. You don't have to have a zoom account to access the meeting. For iPad download the app can be found at zoom.us/download. If you are using a desktop or laptop you don't need to download the app. At this time, a link for meetings will be sent to your cellphone at the beginning of our scheduled session.

If you are a new patient to Holistic Mental Health, PLLC, please fill out the following forms and attach them to a message sent via this link: https://hushforms.com/holisticmentalhealth-nc-contact-dr-ose prior to your appointment or bring them into the office if you are doing an intake in person. For the Medicare opt out form, please fill it out (Yes, even if you do not have Medicare), by printing your name under “Beneficiary Name,” checking each box, initialing to the left of each check box, and sign and date it.
